A 45 year old woman with no known history of cardiac or renal disease and no past history of hypertension complains of one week of upper respiratory symptoms. Vital signs are taken and her systolic blood pressure is 260 mmHg.

1. What is your differential diagnosis?

She is sent to the Emergency Room where she now complains of shortness of breath, but has no chest pain, hematuria, visual changes or headache.

Physical Exam
BP is 190-260/ 90-140, Pulse is 90-140, O2 saturation is 90% on 5L nc, RR is 35, temperature is 36.8 C

Exam is normal with the exception of tachycardia and crackles heard on auscultation of the chest half way up the back.
