An abdominal CT scan was obtained which showed a large fluid filled cyst in the region of the pancreas. Because of the previous history of acute pancreatitis and gall stones the possibility of a pancreatic pseudocyst was entertained.

Two drains were placed within two separate cavities on February 6th. Follow up CT scans and abdominal films were obtained which showed the drains in place. The stomach was thought to be distended with fluid.

CXR showed: “VOLUME LOSS AND CONSOLIDATION IN TRANSPLANTED RIGHT LUNG WITH CIRCUMFERENTIAL PLEURAL FLUID OR THICKENING.”

He received aggressive antimicrobial therapy for his pancreatic abscesses including amoxicillin, Caspofungin, fluconazole, Anzemet and Bactrim. He was continued on cyclosporine and prednisone for his lung transplant, but developed progressive renal failure. The patient died on 2-26 despite therapy.

Fluid from the cysts was cultured but the culture report was not received until after the patient's death.
